For Students - Photo Permissions

As part of Sacred Heart Primary School’s publicity activities there may, on occasion, arise the situation whereby the school, Catholic Education Office (CEO) or local media will need to take photographs and/or video footage of students for publication in classrooms, school newsletters, newspapers, CEO documents, training videos and the school or CEO websites. By signing the consent form below, you can give permission for any or all of these activities to occur with your child for the duration of his/her stay at Sacred Heart Primary School. It will be kept on file for future reference.

Please note that this form will be valid for the duration of your child’s enrolment at Sacred Heart School unless you notify the school of any changes in writing.

For Applicants for Positions at Our School

[The Privacy Amendment (Private Sector) Act 2000 became law as of 21 December, 2001.]

1. In applying for a position you will be providing Sacred Heart School with personal information. The School's contact information is contained on the Web page.
2. If you provide us with personal information, for example your name and address or information contained in your resume, we will collect the information in order to assess your application.
3. You may seek access to your personal information that we hold about you if you are unsuccessful for the position. However, there may be occasions when access is denied. Such occasions would include where access would have an unreasonable impact on the privacy of others.
4. We will not disclose this information to a third party without your consent.
5. If you provide us with the personal information of others, we encourage you to inform them that you are disclosing that information to the school and why, that they can access that information if they wish, and that the School does not usually disclose the information to third parties.
